Update PAT revoke modal under admin credentials and user settings to Pajamas component

Summary

Under Admin > Credentials > Personal Access Tokens and User Settings > Access Tokens, when a user clicks on Revoke they are presented with a system modal confirming the action.

Improvements

This modal component needs to be replaced with the one in Pajamas, and the copy should be updated to provide better context.
