Move PivotalTracker.com story to Done when merging GitLab merge request

• The existing integration with PivotalTracker.com with a GitLab project will move a PivotalTracker.com story to Done when a the story is mentioned in the GitLab commit.
• Extend this functionality so that the same workflow can happen via the merge request description.
• At a high-level, we want to fulfill this use case:
  • Mention the PivotalTracker.com story in the merge request description.
  • When the merge request is merged, automatically move the story to the Done state.
• It would be great if in the merge request description, we can differentiate between GitLab issues and PivotalTracker.com stories, so that both can be used simultaneously. But this isn't a hard requirement.
