Add descriptions to web hooks

Especially when dealing with multiple web hooks per project, or even per branch, the ability to add a description to them like "Foo web hook", "Bar web hook" would be useful.

A web hook's purpose can't always be determined by the url they are pointing to.

The description should be shown in the list of web hooks.
