Add unsubscribe link to plain text e-mail

Summary

Unsubscribe links for comments only appear in the HTML e-mail, not in the plain text.

Steps to reproduce

• enable e-mail notifications
• subscribe to an issue
• wait for a comment
• switch e-mail program to plain text
