Using Closes X, Y in a MR adds a mention for X but not Y

In https://gitlab.com/gitlab-org/gitlab-ee/merge_requests/692, I mentioned two issues, X and Y, but only X was mentioned for some reason in the issue: https://gitlab.com/gitlab-org/gitlab-ee/issues/934

It's possible the worker that created the second note failed for some reason.
