Option to make issues confidential by default

Being able to enforce that all issues in a repository are private would be of great use to me for certain repositories.

Additionally a 'confidential by default' option could be useful for eventually releasing issues as unconfidential but keeping them confidential initially.

If I had to pick just one I'd pick the former enforcement setting.
