Refactor .btn

According to button.scss line 68 we have destroyed the bootstrap button by merging .btn-default into .btn. There are other things about .btn that need fixing as well like the excessive use of &.btn. To fix this issue we take btn-default out of btn and any place that uses the rule btn without another style should be replaced with btn-default. cc @iamphill @JobV bV @alfredo1
