Improve description of commits making changes to issues

I believe that it's important to see commit message when it is referenced in an issue. I think it would be better that instead of:

Status changed to closed by commit d3adb33f

And having to hover the commit hash to see Commit - Author: message, it could be:

Status changed to closed by commit d3adb33f - message

No tooltip required, author is already identified in the issue stream.
