Use Modal instead of dropdown for metric chart alerts

We need to iterate toward cleaner, more consolidated chart options to reduce visual clutter and ease of use.
Proposal
The alerts dropdown should become a modal instead to support our goal of having alerts as one option in a dropdown of chart options

What does success look like, and how can we measure that?
This shows the alert creation form in a modal. For this iteration we don't have to worry about the Add another metric group button as that will be added in a future iteration when we are able to support multiple alerts.
