Merge Request upon merge adds commits to code but the Merge Request stays open

On multiple merge requests we have merged in the Merge Request, which succeeds in adding the commits to the code base, but fails to transition the merge request to MERGED. This causes there to be an open merge request with no commits in it. This is ruining our ability to track what is going into our code base on a Merge Request basis.
