[VSC] auto format document edits as middleware

Problem to solve

To ensure higher quality results fomr Duo Workflow / Duo Agentic Chat, we should leverage the IDE capabilities to auto-format edited code.

Proposed solution

Add a new ApplyEdit middleware. Use the TextEdit ranges to format specific selections. Apply formatting before we save and show the diff.
