Update docs with process for dealing with breaking changes

Our documentation currently does not contain any information around breaking changes / major version bumps. We currently say

1. we haven't release major version yet, there is no process for that

As we're currently on version 5.x I think this is not correct :)

Proposed solution

Add some initial guidance on dealing with breaking changes, gotchas around squash commits etc
