Voluntary product accessibility template - review and update

At the Summit I spoke with Brent about accessibility and the template they use on a Federal level to complete the VPAT (https://www.section508.gov/sell/vpat). I've attached it here. We should review the state it was in a year ago (Jan 4, 2017) and see what needs updating. It can also help serve as a guide for where we can improve.
