Unable to create new Group

I was on 7.7.2 & recently upgraded to 7.8. I am unable to create New Group in both versions. On new Group page I dont see Group Name text box and on clicking Create Group button it complain about "Name cant be Blank".

See the screenshot below. Tried on chrome and firefox with Admin accounts.
