Transparently proxy SSH Git push from secondary to primary - improvements

Building on from https://gitlab.com/gitlab-org/gitlab-ee/issues/6533, there are a number of improvements:

• Move proxy code from gitlab-ee into gitlab-workhorse
• Add QA scenarios to thoroughly test all use cases
• Update gitlab-shell to more accurately handle the new HTTP status codes sent
