Add ability to create a note for users (also when blocking them)

Requested feature
It would be very useful to be able to make a notation of the reason when you're blocking a user. This would help to deal with issues if they come back to you and ask for their access again. If you deal with a lot of users it can be useful to remember why you blocked them.
