Adds an issue template for use when rolling out feature flags

In https://gitlab.com/gitlab-org/gitlab-ee/issues/9672, https://gitlab.com/gitlab-org/gitlab-ee/issues/9657, and https://gitlab.com/gitlab-org/gitlab-ee/issues/9654 I was doing something similar but copy pasting each time.

Why

After https://gitlab.com/gitlab-com/gl-infra/infrastructure/issues/6750 there were some ideas to improve the workflow and because having a template is better than copy pasting each time.

There is some more formal discussion relating to this in https://gitlab.com/gitlab-org/gitlab-ce/issues/59705 and https://gitlab.com/gitlab-org/release/framework/issues/335, so I expect we might iterate on this in response to discussion there.
