Distributed code-reviews and issues

Store code reviews as git notes with Git Appraise see:

Apart from that I think there is also room to make issues truly distributed. Fossil already did this http://fossil-scm.org/index.html/doc/trunk/www/bugtheory.wiki

All this is pretty far out though.
