Instrumentation for on-demand usage acceptance flow

Proposal

Create a funnel for overage acceptance flow:

1. How many customers had at least 1 user blocked due to credit limit
2. How many customers from 1 above were sent on-demand acceptance email
3. How many customers from 2 above clicked on the acceptance link in email
4. How many customers clicked on the accept on-demand terms in the UI
5. How many customers actually accepted on-demand terms
