Add documentation on upgrades and known limitations

Let's touch up the docs with:

• How to update the Operator
• Known limitations/caveats

The end goal is that end users can deploy and upgrade the Operator by following the documentation, and can be aware of limitations and/or configuration changes required for this version of the Operator that may differ from a standard Charts installation.
