Clarify process for planning and scheduling work vs unplanned emergent issues

Why is this change being made?

Previously we had multiple references to planned, scheduled work as "unplanned", which is confusing. This change attempts to clarify the definition of unplanned work, and sets an initial target capacity for various types of work in each milestone.
