Stabilize pages with tables of contents, O-V.

This should eliminate content shifting related to tables of contents in the remaining handbook sections with names that start with "O" through "V". It changes the layout for these pages to handbook-page-toc, which includes the sidebar, and it adds responsive hiding classes to the table of contents elements in the Markdown source so these things don't have to be added via JavaScript after the page has already been loaded.
