Feedback for Knowledge Articles

Need: A way customers can provide "Comments" for Feedback on knowledge articles.

Currently, at the bottom of each Knowledge article page, we offer a " was this helpful?" Yes or no option.

Requested Change:

  1. Add a Comment Box that populates on choice

    Yes - Great! do you have any feedback for us?

    No - We want to make it right! Let us know what we can do to fix this.

Comments should be tracked "Somewhere" (Ticket? Issue?) and assigned to the Knowledge base for tagging
