Introduce a new Internal Request form for upgrading existing Premium SaaS customers to an Ultimate trial
Problem Statement
The long-awaited EPIC Allow existing SaaS Premium customers to trial ... (gitlab-org&9549) is currently being implemented by the Fulfillment team. As part of this overall EPIC, a first iteration EPIC Iteration 1: Allow internal teams to provide ultimate trial has been created which will allow internal GitLab teams to upgrade an existing Premium customer to trial an Ultimate subscription.
Account Executives will be required to open a support ticket with L&R support to facilitate a Premium Subscription Customer trialling an Ultimate subscription as agreed in the following issue How should GitLab handle the impending changes which will allow SaaS premium customers to trial GitLab Ultimate?
What is the problem?
No current Internal Request form exists for this new process
Why is this a problem?
Account Executives need a familiar mechanism to submit a ticket for this functionality and support needs to accurately process these requests via ZD.
Proposal
Create a new internal form, the form should be almost identical to the form named Change Existing SaaS trial plan
only without the drop-down menu Desired new plan
given that the new trial plan can only be Ultimate.
