Migrate to one "super" form

Need

Through the years, we have developed several forms and methods for getting assistance, such as:

  • Issue in organizations repo
  • Global IR form
  • US Fed IR form
  • Gratis Support form

This has become messy to manage and has made answering "where do I go to do xyz" difficult to answer.

So we are working on a "super" form, but we'd like to get feedback and comments on it (keep in mind it does not allow submissions at this time).

Approach

Migrate our many different areas into one "super" form. Get feedback. Slowly migrate over to using just that.

Benefit

Turns a complicated answer into a simple one:
