Create 2 new GitHost infrastructure VMs for migration

We're prepping to move/rebrand GitHost to GitLab Hosted. We need one server for MySQL and one for the Rails application.

• Rails server: 8GB instance. Nothing pre-installed.
• MySQL server: 8GB instance. MySQL pre-installed and hardened. Ideally port 3306 should be allowed only from the app server.

I don't particularly care what the hostnames are, but they should probably be provisioned in the gitlabhosted.com domain.
