Create survey to measure the maintenance policy soft launch's impact

Context

The soft-launch of the policy extension https://gitlab.com/gitlab-com/gl-infra/delivery/-/issues/21474 is aimed to enable engineers to self-serve bug fix MRs to maintained stable branches.

We should monitor the engagement data for 1-2 milestones, then measure impact on engineering and gather internal feedback. With that data, we can have a better idea on the impact of the extension, present it to leadership, in order to officially roll out the extension.
