Conditionally include `terraform-module-publish.yml` in the
terraform template, gated by a new `include_module_publish`
boolean input (defaults to `false`).

New inputs on the terraform template:
- `include_module_publish` (boolean, default: false)
- `release_stage` (default: release)
- `module_dir` (default: .)
- `module_name` (default: ${CI_PROJECT_NAME})
- `module_system` (default: "")

Also updated the terraform template README with documentation
and a usage example for the new publishing feature.
