feat: BackendRegistry integration with Spring Cloud Kubernetes

- POST /api/runs gains 'backend' field for named backend resolution
- GET /api/backends endpoint lists configured backends (for web UI)
- Spring Cloud Kubernetes: native ConfigMap discovery + hot-reload
- @ConfigurationProperties with Bean Validation (schema check at startup)
- @RefreshScope: ConfigMap changes take effect without pod restart
- fhir-frog-server dependency for shared BackendRegistry
- Secrets via K8s secretKeyRef → env var interpolation in config

- Dockerfile for multi-arch container builds
