Settings page: Don't show filter list title element if no title is passed via subscribe link

Background

It's not required for subscribe links to include a filter list title. Therefore we should not show any title in the confirmation dialog that shows up when clicking on one such link.

Goal

Remove the filter list title in the confirmation dialog as specified in https://gitlab.com/eyeo/specs/spec/merge_requests/151.
