As a visitor whenever i click a link it takes me somewhere, not confusingly re-loading the page i'm on

This is primarily about content titles (aka node titles). We should end Drupal's annoying habit of having a piece of content link to itself from the title— but we should incorporate a clearly-identified permalink icon.
