Same name and DOB causes failure

From community partner: "Noticed another thing while trying out the app. If there are multiple entries with the same name and DOB, the system gets confused and stops running."

They went on that in these cases, when they are gathering the information manually, they would "We would just click the search results and view the information." and then sort it out by hand. I wonder if we should handle this the same way? Just combine it all into one CSV?

I'm requesting an example to follow up and see where the failure is and what can be done to move past it.
