Add ability to create firewall rules as disabled at time of creation.

When staging in a new box, it would be useful to be able to provision firewall rules as 'disabled' at the time of creation. This is especially useful for virtual addresses and 1:1 nat rules which may provide contention with other network devices if the new box is provisioned along side the old box.

The feature already exists with Custom rules. Not for any of the others.
