user table addition and privacy messages

• Add a first name and a last name fields in the user table and reflect this in the account creation page and edit pages for users (admin and user versions). I need this to more easily trace who is who ( or who they want me to think they are anyways).
• Add wording with a check box to be seen when creating an account "Privacy warning: by creating an account, you accept that we store your email address and your name. You can write to cdli@ames.ox.ac.uk to have your account deleted at any time. You understand that we will link your account to a public author profile. This profile will display and credit you for any suggestions you provide cdli regarding text, text annotations, images, images annotations, and metadata.
• Add wording in the user facing user profile page: "You can write to cdli@ames.ox.ac.uk to have your account temporarily deactivated, or permanently deleted, at any time."
• Above the update event form "Privacy warning: by submitting your suggestion(s), you acknowledge that your name will be associated publicly with it once it is accepted."
