Second refactoring pass: content expansion and consolidation.

New content:
- Policy Handbook (655 lines) with background, rationale, testability
  criteria, and practical examples for all 16 policies
- Adoption directory: webmaster guide, HTML page template, footer snippet
- RFC-0002: Human-Readable Disclosure (non-normative)
- Spec Sections 16-19: Disclosure page, footer signals, badges, llms.txt/ai.txt
- Standalone llms.txt and experimental ai.txt examples

Improvements:
- Terminology glossary expanded from 10 to 18 terms
- Schema versioning policy with breaking/non-breaking change rules
- Registry non-enforcement language clarified
- GOVERNANCE.md extended (Sections 2.4, 2.5, 7.4)
- Policy count corrected 15→16 across all files
- Redundant policies.md removed, consolidated into handbook
