Why does the tool report multiple mutated lines in a single file (mutant), when it is expected to report only a single mutant?

I used the Mdroid tool to generate mutants for the OmniNotes application available on GitHub. However, upon comparing one of the mutated files with the original file, I observed several changes in that file (Several mutants). This was unexpected as I had anticipated only a single change. Figure below show comparison of original and mutated file.
