Cannot enable WiFi calling on Pixel 4 XL after 20 -> 21 upgrade.

Expected Behavior

Toggling WiFi calling from 'OFF' should launch relevant carrier service app and turn the feature to 'ON'.

Current Behavior

Toggling WiFi calling from 'OFF' attempts to launch app but the app immediately closes/crashes, sometimes showing an ANR dialog.

Steps to Reproduce

On a Google Pixel 4 XL, clean install of the most recent LineageOS 21 build, attempt to enable WiFi calling.

Supplemental

WiFi calling on this device has been working at least since the release of LineageOS 20, if not before. It has been enabled and in active use for at least 3 years. During that time the phone has been wiped and reinstalled at least twice without an issue re-enabling WiFi calling. I believe I have eliminated every possible cause other than a bug in LineageOS itself.
